Make appointment of Dentist or a Laywer.

Document Checklist:
1. Current PR card
2. Copy of Passport
3. Copy of Record of Landing (IMM 1000)
4. Proof of Marriage Certificate
5. Mary’s citizenship Card
6. Two Photos (Will have in Toronto)
a. One photo on the sticker of the Supplement Identification Form (IMM5455)
b. Another photo will be put in a small envelope and signed by the Guarantor.
7. Forms: (b & c will be get in Amy’s home, already ordered)
a. Application form for a PR card (IMM5444, you can download from www.cic.gc.ca)
b. Supplementary Identification Form (IMM5455, you need to order first and cic will mail to you), with the photo correctly attached and signature in black ink inside the white box.
c. Copy 2 of your fee payment receipt (IMM5401, you need to order first and cic will mail to you)
d. Use of Representative (IMM5476, you can download from www.cic.gc.ca) (If Applicable)
e. This Document Checklist (IMM5574, you can download from www.cic.gc.ca)



Day1:
Take photos in Pacific Mall.
Go to Dentist Ding Jie to get the signature or go to see a laywer.
Go to TD bank pay the $50 CAD application fee with fee payment receipt (IMM5401)
Mail out everything (including Application form, paid slip, photos and so on)

Day2:
Go to take the Driver’s License test
