I completed mine 4 years ago and I know since then they have made some changes to it so you should really check with CGA. I can tell you that when I was eligible to complete the PERC I had access to a link from CGA Website and when I clicked on it there was plenty of instructions that accompanied it. There were many sections to go through to check off make comments when my boss went through and approved it. There is also a large section for you and your boss to add anything that you feel is relevant. Good luck!
